There are a lot of details to consider when looking for the best placement since each situation and person is unique.\n \nWe are going to cover:\n priorities of a hospital or rehab facility when it is time to discharge types of residential placements for older adults needing assistance levels of care -ways to pay for placement or care (Long-term care insurance, ALTCS, VA, reverse mortgage) how to find the best placement for your clients websites, placement agents -alternative ways to have care and stay at home things to consider before you commit \n \n \nAbout the Speaker\n \nBeth Fuller, BSW, CMC, founded TLC Coordination and Advocacy, a private care management company in 2016. She is an advanced professional member of the Aging Life Care Association™ and certified by the National Academy of Certified Care Managers. She has provided care management services to older adults, and individuals with developmental disabilities or a serious mental illness for 40 years in Tucson.\n \n \n \nAbout the Panel\n \nNan Lagemann, LMSW, CCM is a certified case manager and licensed social worker. and has over 20 years of experience working in hospital settings.  She also ran a program for older adults experiencing later life domestic violence in Pima County for 7 years.  Since 2017.
